3. Payment of R$ 200 to the Embassy’s account below:

4. A bank slip (Comprovante) should be submitted in person during the Mobile Consular Services exercise. No refunds will be made for applications mailed to this office with amounts over and above the indicated amount.

5. Two clear copies of the original birth certificate or birth notification from hospital.

6. A pre-paid self-addressed return SEDEX envelope. The envelope should have sufficient return postage.
